We no longer offer paper forms. Instead, we are implementing a digital registration and check-in system that will streamline your check-in process. When you schedule a visit, you will receive a link to complete your check-in before arriving for your appointment. If you are unable to complete your check-in in advance, please arrive 15 minutes prior to your scheduled appointment time to allow us to assist you with check-in.
